I will make the argument that the best way to reform the institutions is to increase the awareness and desire for change of enough individuals that the reform becomes a majority issue.
It IS up to individuals to train the police force on rape kits and counseling... on the individuals creating the training materials and doing the training. And same for all the other examples. And if those have enough other individuals telling them this is important, it will happen.
