And of course, humans were in the loop and ignored the warning email. So I'm not sure that (people "in the loop") is much of a defense for these kinds of problems.

I have a sneaking suspicion that the email notices were trapped by spam filters. Claiming that the people "ignored" the emails assumes that they actually saw and read those emails. I don't see or read many auto-generated notifications, myself, because 99.99% of them _are_ spam.
