Ask the Friends: Rec letters?

Does anyone have a good guide or preferred how-to on how to write a good recommendation letter? As usual, doing general Googling gets you all sorts of advice, much of it contradictory. I'm starting to think Google is run by elves. Or maybe it's just that elves maintain the Web's supply of advice pages...

See if Ask A Manager has any advice; theirs is generall good IMO/IME.
